Ko'olau Radiology Is Contracted With Most Insurance Providers
Our referral coordinators assist you and your health care provider with insurance verification and prior-authorizations necessary for your exam. Ko'olau accepts all patients and bills all insurances. Your financial responsibility for your exam or procedure will depend on the type of insurance plan you have and the individual contract Ko'olau has with your insurance company. We urge you to contact your insurance provider for more information about your individual coverage.

Is this exam covered by my insurance?
If you have a deductible: Have I met my deductible already or will I be responsible for some or all of the cost for this exam?
If you have co-insurance: What percentage of the exam fee will I be responsible for?
If your exam is not covered by insurance:
What does the exam cost if I am responsible for 100%?
Are all fees, including the professional fees, technical fees and any facility charge included in this price?
Am I eligible for any discounts? What if I pre-pay the entire amount or a portion of the amount of the exam?
Will I receive one or several bills for my exam?
Ko'olau Radiology Credit Policy
The best medical care can be provided only on the basis of mutual understanding. We, therefore encourage our patients to discuss any questions you may have regarding our policies.
To avoid misunderstandings, our business office invites early discussion of financial problems or questions regarding fees or payment from insurance carriers. Our requirements for maintaining your account in good standing are as follows:
All charges are due and payable within 30 days of statement date. Under certain circumstances a payment in advance may be required.
Other circumstances may warrant a payment plan; this will be set up with the Business Manager.
A fee of $25 will be charged for all returned checks.
All services are subject to a state of Hawaii Excise tax of 4.712%. An itemized statement covering all medical services received will be mailed to you on a monthly basis. Contracts that Ko'olau Radiology, Inc. has negotiated with some insurances will affect the total charges and taxes incurred. Adjustments, if any, will be reflected in your monthly statement.
This office cannot provide professional courtesy discounts as part of our compliance policy with the federal government. We cannot accept the responsibility of negotiating claims with insurance companies or other persons. The patient is responsible for payment of medical care within a reasonable time (regardless of status of a claim).
Your insurance policy is a contract between you and your insurance company. It is important that you understand its provisions. We cannot guarantee payment of your claims. If your insurance company pays only a portion of the bill or rejects a claim, any contact or explanation should be made by you, the policy holder. Reduction or rejection of your claim by your insurance company does not relieve the financial obligation you have incurred.
